3-on-6 is a full-arch restoration — meaning it replaces all of the teeth in an upper or lower arch (or both). Candidates are typically adults who are missing most or all teeth in at least one arch, or who have failing, periodontally compromised teeth that need to be removed. Strong candidacy looks like this:

Many patients worry that a health condition automatically rules them out. In most cases, it does not — it requires thoughtful planning.
Candidacy for 3-on-6 cannot be determined by a phone call, a photograph, or a form. It requires a clinical evaluation with 3D imaging. Here is what that looks like at Significance Dental Specialists:
A good candidate for 3-on-6 is an adult with most or all teeth missing from at least one arch, sufficient jawbone density to support six implants, and no uncontrolled systemic conditions that impair healing. Patients currently wearing dentures or those with failing teeth are among the most common candidates. Candidacy is confirmed through a 3D cone-beam CT scan, which Dr. Huang uses to assess bone volume, anatomy, and nerve position before creating a treatment plan.
Moderate bone loss does not automatically disqualify you for 3-on-6. Many patients with bone loss are successfully treated after bone grafting, sinus lift surgery, or ridge augmentation — procedures Dr. Huang performs routinely. Severe bone loss may require an alternative full-arch protocol, but this determination can only be made after a 3D cone-beam CT evaluation. Patients told elsewhere that they cannot receive implants are frequently candidates after proper evaluation.
Controlled diabetics and non-heavy smokers are often successful 3-on-6 candidates with appropriate medical management. Well-controlled diabetes (HbA1c below 7–8%) does not significantly impair implant outcomes. Smoking increases failure risk and slows healing but does not automatically disqualify patients — risk is discussed honestly during consultation. Uncontrolled systemic disease affecting bone metabolism is reviewed during medical history intake.
Yes. A board-certified periodontist with advanced bone-grafting and implant expertise sees cases that general dentists or less experienced specialists appropriately decline to treat themselves — but that does not mean the patient is not a candidate.
